idiot

Why am I such a fucking idiot? I feel so stupid every day that I'm in internal medicine. Ugh. I sit there and i'm asked questions and I just am SO BLANK. I used to be smart, what happened? Maybe I should go into obs/gyn, it's the only thing I've actually felt intelligent at.

You've advanced in your career and the questions have got harder. You are smart - the people questioning you are just more experienced. Positive self-talk is really what's called for at this point! (Meanwhile I'm pretty much feeling the same :) )

Don't beat yourself up! I remember my third year medicine rotation. For the first three weeks I walked around absolutely terrified and completely baffled. All of the patients are so complicated and have a billion things wrong with them. You eventually start to recognize patterns and things get easier. By the end of the rotation you'll be shocked by how much you've learned.
